Tenet Raises Outlook but Flags 2027 Adjustments

Chairman and chief executive officer Saum Sutaria raised full-year adjusted EBITDA guidance to $5.05-$5.2 billion, representing a $195 million increase at the midpoint.

Chief financial officer Sun Park projected 2026 revenues of $22.05-$22.45 billion and adjusted earnings of $22.05-$23.01 per share. Adjusted free cash flow guidance increased to $2.825-$3.125 billion.

THC Ambulatory Growth Benefits From Higher-Acuity Care

United Surgical Partners International (USPI) generated adjusted EBITDA of $537 million, increasing 9.1% year over year.

Same-facility revenues increased 4.5%, supported by 6.1% higher revenue per case, despite a 1.5% decline in surgical case volumes. Total joint replacements increased 12.8%.

Responding to Deutsche Bank, Sutaria identified orthopedics, urology, spine and advanced robotics as important growth opportunities. Management continues pursuing acquisitions, partnerships and new surgical centers.

Hospital adjusted admissions increased 3.2%, while segment adjusted EBITDA reached $818 million.

Park highlighted sustained expense management and technology-enabled productivity improvements. Exchange admissions and revenues declined 18%, creating an unfavorable payer mix.

During questioning from Guggenheim, Park emphasized favorable commercial contract renewals, generally ranging from 3% to 5%. More than 90% of next year's commercial contracts were already secured.

THC Addresses Conifer's Nonrecurring Benefits

The accelerated transition of CommonSpirit revenue cycle management services contributed $147 million to third-quarter adjusted EBITDA.

Park explained that $97 million reflected revenue recognition timing, while approximately $50 million came from transition-related efficiencies.

Responding to Leerink Partners, Park clarified that the timing benefit shifted revenue from the fourth quarter into the third. Management expects the broader $450 million transition contribution to create a comparison headwind in 2027.
